Adding a 'why are you unsubscribing' form

Hello,

We would love the option to include a 'why are you unsubscribing' form to the email unsubscribe flow itself. Is there a way to do this, or something similar? We want to know why people want to opt out when they click unsubscribe within an email campaign without them having to use a separate form (which no one will want to fill out if they don't have to, hence wanting to make it part of the existing flow).

Any suggestions please? Thanks!
