How can I add a check field to accept terms & conditions within web forms?

I did create a lead web from, and it's missing a check box for clients to check and accept terms & conditions, and additional one to subscribe to newsletter. How can I add these check boxes to my web from

    Hi @Muneera 

    You could use a single option type of custom field with value yes and no for both of these.
    In the text you can then add a link to the terms of service and ask for them to opt-in to both of those.
