Form with info from Pipedrive (not to Pipedrive)

I want to create a work order form per deal populated with various custom fields we have in Pipedrive. The form needs to have a yes / no response which updates a corresponding field on the deal in Pipedrive.

All the forms I have looked at are for gathering information to add to PD, we already have the information in PD and want to share it via a form and seek a response to the information (yes/no)

I already responded to this question on the FB group, but if you want to have a form in place, you can check out Fillout, Tally, MakeForms, or JotForm.

You can create pre-filled forms (URLs inside Pipedrive), and then once a form has been submitted, tie it back to Pipedrive using Make/Integromat or Zapier.
